Sehome Eye Care is happy to accept patient referrals. Our team is dedicated to diagnosing and treating eye health concerns for everyone.

Who to send

We are a full scope optometric clinic. We have subspecialties in urgent care, myopia management and specialty contact lenses, but routinely see patients for anything within the optometric scope of practice. Our daily routine includes everything from primary annual eye exams, to dilated diabetic exams, glaucoma management, plaquenil screenings, and everything in between.

What to send

All we really need is the patient’s name, phone number, and reason for referral. Feel free to send whatever notes you have, but no special forms needed.

What to expect after the referral

We always stay in contact with referring providers. Expect a letter back either after we’ve seen the patient, or after we’ve reached out to them at least three times.
